The resolution for this would be to take off the toilet tank and replace the tank bolts with new gaskets. Since the tank is already off it makes sense to go ahead and replace the flush valve with a new tank to bowl gasket. If the fill valve and flapper are showing signs of wear or have not been replaced in a while, this would be the time to replace those as well. This is what I call a complete toilet tank rebuild.

This is the ballcock style fill valve and a standard flapper. These are the signs of wear I would be referring to in determining if I needed to replace the fill valve and flapper. Also I can tell the tank bolts are in need of replacing.

More than likely the tank to bowl connections were tightened too much. Over time this caused enough stress to crack that area and allow it to break. It could also be a combination of things. Possibly someone leaning on the toilet tank like a recliner. The resolution for this would be to install a new toilet, both the tank and the bowl.

The video is of a pin hole leak that formed in the copper pipe connecting the shower valve to its diverter valve. The resolution would be to take each valve out. Sweat out the piece of copper between, and sweat a new piece in to connect the two. Then install the assembly back in place in the wall behind the tile.
